Additionally, you will receive a Health Check trophy after completing your first Health Assessment. You should be able to see your points being reflected, after completion. To confirm your Health Assessment completion, please navigate to the Home > Rewards page. In case your company allowed frequent reminders, you will also be reminded via email and/or via pop-up window that will regularly show up on your screen, until you finish completion of the Survey. That way you will most certainly have some extra time and energy to finish more activities that interest you too. Try to find few minutes of your time to spare and complete this activity in early stage of the program. Keep in mind that it will stay there until this activity is completed. If you have not yet completed it, you will see one more Daily Card in available ones, which should indicate that you should complete your Survey. Still and all, please keep in mind that you will only be rewarded once per program for this specific activity. Health Assessment survey allows you to re-take it at any point for as many times as you like. You will be redirected to page from where you can start your Health Assessment.
